By offering free, 30-minute consultations, our hope is that you can have some time to share your medical and dental history in a comfortable place while receiving a specialty opinion. After hearing what we believe your options might be moving forward, we ask that you go home and think things over. Once you feel fully ready to make a decision, Restoration will be there waiting.
In order for us to best serve you, you will receive a packet of forms prior to your first appointment. Please, fill the forms out to completion (as best you can) before arriving. Should you have questions or need assistance, we will be happy to help you when you are in the practice. Those forms are available online through our patient portal or as a downloadable and fillable PDF. If you choose the fillable PDF, you can either print the forms or send them electronically.
While there isn’t a fee for your consultation, it is important for you to review our financial policy and insurance policy prior to your arrival. We try to accept as many forms of payment as is possible, but to ensure there aren’t any hiccups in the process, make yourself familiar with those policies, so we can answer any questions you might have.
